ABSTRACT

Se presenta el caso de un niño de 5 años sin antecedentes de enfermedad, que se internó en terapia intensiva por convulsiones tónico-clónicas focalizadas en la cara y en el hemicuerpo derecho, con documentación de temperatura axilar de 37,4°C. Se descartó la presencia de gérmenes comunes y la etiología viral a través de estudios de muestras de líquido cefalorraquídeo (LCR). Se sospechó la presencia de Mycoplasma pneumoniae por comprobarse inmunofluorescencia positiva en suero para anticuerpos de clase IgM. El diagnóstico se confirmó mediante la detección del ADN de dicho patógeno sobre la biopsia cerebral efectuada por el método de la reacción en cadena de la polimerasa (PCR) y una histología compatible con encefalomielitis aguda diseminada. El paciente recibió tratamiento con claritromicina y su evolución fue favorable. Al menos dentro de nuestros conocimientos, este es el primer caso en el que se detectó ADN de M. pneumoniae en una biopsia cerebral por el método de PCR.


We present here the case of a previously healthy 5 year-old boy hospitalized in an intensive care unit due to tonic-clonic seizures focused on the face and right side of the body, and axillary temperature of 37.4 °C. Common bacterial and viral etiology was ruled out through studies of cerebrospinal fluid (CSF) samples. Mycoplasma pneumoniae was suspected by a positive immunofluorescence serum test for IgM class antibodies. Finally, with a brain biopsy, M. pneumoniae was confirmed by polymerase chain reaction (PCR) and acute disseminated encephalomyelitis by pathological anatomy. The patient was treated with clarithromycin and had an uneventful evolution. At least to our knowledge, this is the first case in which M. pneumoniae DNA was detected by PCR in a brain biopsy.

ABSTRACT

Disseminated encephalomyelitis (ADEM) is an infrequent condition with considerable morbidity and mortality in adult patients. It requires a high level of suspicion and diagnosis emerges by gathering clinical information, laboratory exams and images studies. ADEM is related to an immunological phenomena occurring after a bacterial/viral infection or recent vaccination. Glucocorticoids are the first line treatment, reserving immunoglobulins and plasmapheresis to refractory cases. We report a male patient aged 25, with ADEM associated to parainfluenza 3 virus respiratory infection that required mechanical ventilation and that had a complete recovery only after plasmapheresis.


La encefalomielitis aguda diseminada es una enfermedad infrecuente pero de elevada morbi-mortalidad en pacientes adultos. Demanda una sospecha y diagnóstico precoz que requiere el concurso de información clínica, pruebas de laboratorio y estudio de imágenes. De sustrato inmunológico, se puede relacionar a una infección viral, bacteriana o inmunización reciente. Los glucocorticoides son el tratamiento de elección, mientras que la inmunoglobulina intravenosa y la plasmaféresis se reservan para casos refractarios. Se presenta el caso de una encefalomielitis aguda diseminada grave, en un paciente de sexo masculino de 25 años, asociado a una infección respiratoria por virus parainfluenza 3. Requirió conexión a ventilación mecánica y tuvo una respuesta completa con plasmaféresis.

ABSTRACT

Acute disseminated encephalomyelitis [ADEM] is one of the demyelinating disorders of central nervous system [CNS] causing acute or relapsing-remitting encephalopathy. We report a male child, who presented with fever, fits and altered sensorium, with two identical events, 3 1/2 months apart. Neuroimaging showed high signal lesions on the T2 weighted images reflecting areas of demyelination and edema, consistent with ADEM. He responded with complete recovery after pulse steroids and intravenous immunoglobulin [IVIG] therapy

ABSTRACT

Acute disseminated encephalomyelitis (ADEM) is a demyelinating disease of the brain and spinal cord which is preceded by many viral and bacterial infections. We present a rare case of ADEM following falciparum malaria infection. Early identification of ADEM is highly required as it responds to treatment very well and delay in diagnosis may lead to permanent neurological sequelae.

ABSTRACT

La encefalomielitis aguda diseminada (EMAD) es la leucoencefalopatía adquirida de mayor presentación en la edad pediátrica, de naturaleza inflamatoria-autoinmune, generalmente monofásica, polisintomática, y asociada a compromiso del estado de conciencia. Frecuentemente es precedida por un proceso infeccioso o por una inmunización. Se revisó la literatura médica de los últimos años considerando los aspectos etiopatogénicos, clínicos, diagnósticos y terapéuticos. Se presentan los últimos criterios diagnósticos para la EMAD y sus formas recurrentes, así como la importancia de los diagnósticos diferenciales, entre ellos la Esclerosis Múltiple Pediátrica(EMP). Las imágenes de Resonancia Magnética (IRM) constituyen el estudio de elección para detectar lesiones desmielinizantes.


Acute disseminated encephalomyelitis (ADEM) is an acquired leucoencephalopathy, that occurs more frequently during childhood, of inflammatory autoimmune condition, usually monophasic, polysymptomatic and associated with sensory deterioration. It is a frequently preceded by infections or vaccinations. Recent medical literature was reviewed considering etiology, pathogenesis, clinical, diagnosis and treatment aspects. The current diagnostic criteria for ADEM, its variants and the relevance of differential diagnostics, like Pediatric Multiple Sclerosis (PMS) are included. Magnetic Resonance Imaging (MRI) is recommended to detect demyelination injuries. High doses of steroids are still the therapeutic alternative for ADEM.

ABSTRACT

La encefalomielitis diseminada aguda es una patología poco frecuente, pero de gran impacto en salud pública. Se sospecha clínicamente y diagnóstica en base a los hallazgos de estudios imagenológicos, como resonancia nuclear magnética. En este artículo se realiza una revisión de la literatura para orientar al pediatra en caso de sospecha del diagnóstico.

ABSTRACT

Encefalomielitis postinfecciosa o encefalomielitis diseminada aguda, constituye una enfermedad que sigue a una variedad de infecciones virales y/o bacterianas. Su verdadera incidencia se desconoce, sin embargo en los Estados Unidos se reporta entre un diez al quince por ciento de todas las encefalitis virales. El número de casos disminuyó notablemente con la eliminación de vacunas a virus vivos. Clinicamente resulta imposible de diferenciar de las encefalitis, principalmente la herpética. La RNM con gadolinio es el método de diagnóstico de mas ayuda a la hora del diagnóstico diferencial, con las encefalitis virales, ya que es el único método que distingue claramente el compromiso de la sustancia blanca.

ABSTRACT

A ocorrência de desmielinizaçäo inflamatória do sestema nervoso central ou periférico, após doenças infecciosas agudas ou vacinhas, é reconhecida há longo tempo. Relata-se caso de Encefalomielite Disseminada Aguda (EDA), em pacientes de dez anos, após vacinaçäo em campanha de saúde pública anti-sarampo. Discutem-se os aspectos clínicos e imunopatológicos dessa afecçäo e, apoiando-se na literatura especializada, propöe-se uma estratégia de manuseio de EDA, fundamentada em suspeita precoce, diagnóstico e rápida instituiçäo de medidas terapêuticas apropriadas. A utilizaçäo de metilprednisolona em pulso encerra a manobra inicial, além de medidas de suporte clínico; plasmaferese e imunossupressores constituem opçöes complementares, em casos mais graves. Medidas preventivas (vacinaçäo contra doenças infecto-contagiosas, por exemplo)e avanços biotecnológicos, com desenvolvimento de vacinas menos imunogênicas, ensejam menor risco de EDA na populaçäo suscetível
